If I used this tool on cardstock (60 lb), would it poke holes all the way through the paper, or just make indentations? Looking for 5 holes per inch.

It might punch holes in paper if you had the paper on a spongy surface. It mainly makes indentations to indicate where to punch holes.

Can it poke holes or just mark borders?

No it will not punch holes. It only marked the spaces. It also can be used for decorative stitch marks.

does it come with a handle

This set comes with a handle as shown , three embossing wheels and a screwdriver for changing the wheels.Worth Jackson/ J J Enterprises, LLC

How deep are the cuts (I need to perforate upto maximum 2mm depth)?

I believe that depends of the pressure you use, but in fact this tool is made for give a guide or to make marks on the surface, not to cut
