How wide and how thick is this tape?

Wide: I'd say 2 or 3mm. Thickness: its quite thin. I bought it because my lower teeth are so tight together, I couldn't get normal dental floss between them without it shredding.

Is this dental tape smooth?

Yes, and it doesn’t shred. I really like it. It moves easily between sensitive teeth, and does a good job.

Is the tape made of waxed thread string or plastic ribbon tape?

I’m not sure, but I would guess plastic ribbon tape. It is very strong like the waxed thread string though.
