So if my dog weighs more than the 40 to 60 lb dose do I do the math to his exact weight or will the two chewables be enough?

Just follow the large dog dosage instruction. Our dog is about 70 lbs, so we gave 1 1/2 tablets. Hope it helps.

Does it also kill mosquito larvae to prevent heartworm?

No. It does not kill mosquito larvae. This is mostly an intestinal parasite wormer.

For the 26-60, how many treatments come in each box? I have two dogs and need to know how many boxes I need for one treatment?

26-60 box contains 2 tablets. The recommended dose depends on dog's weight: 26-30 lbs - 1 tablet, 31-44 lbs - 1.5 tablets, 45-60 lbs - 2 tablets.
