Question 1

The book says to calibrate with the slideouts deployed. Yet elsewhere it suggests keeping the slideouts in. Are the slideouts supposed to be retract?

Pretty simple process. 
Step 1) level the trailer with the slide(s) out just like you normally do.
Step 2) bring the slide(s) in
Step 3) install and calibrate this as level. This will be a bit off true level, however when you put the slide back out, it goes back to the point that it was level (true level) with the slide out

Question 2

Are you selling/shipping the 2nd generation with on/off switch?  it says that you are shipping but want to be sure i am purchasing the gen2.

I have 1st gen and argued with their support for while about it not having an on/off switch.  They made the statement that the average RV only travels once or twice a year.  I explained that the folks willing to fork out this kind of money for this single function might be traveling weekly or daily. PUT A SWITCH ON IT!!!!  Now, they have agreed with me and others, I suppose.  For those with Gen1, I would suggest what I'm going to do...1) Get a 15 cent micro switch and install it on the cover.  2) Get a 3v cell battery (not a button....the larger types you might find in a garage remote or something.    With some small wires, tape, whatever you need, wire in the switch and bigger battery.  Should run as well if not better than the gen 2 if you have a much higher capacity battery.

Question 3

Do you need to purchase The Anderson drove on blocks? Will this work okay with other leveling blocks?

It will work with whatever leveling blocks or jacks you have. It basically shows on a diagram what wheels need to be leveled and by how much. How you accomplish that is up to you.

Question 4

Interested but have a 40ft 5th wheel. where to mount for best results? and also a bit worried about bt connection from truck. will it work?

The best way to determine an optimal mounting location that will give you a reliable connection from the driver's seat of your tow vehicle is to test any proposed mounting location.  Follow the steps below to find a good mounting location.

1.  Install the battery and turn the unit on.
2.  Put a piece of double-sided tape on the back of the unit and stick it to a wall where you are considering mounting it.
3.  Put your tow vehicle a few feet in front of the normal towing position to create a worse case scenario relative to distance.
4.  Test the connection repeatedly from the driver's set of your tow vehicle.  Each connection should be tested for at least 1 minute and you need to allow 15 seconds between the time you close the app and the time you reopen it to connect.  This delay will allow the phone time to properly hang up the Bluetooth connection.
5.  Try multiple locations if necessary.  Generally speaking, toward the front, high on the wall or in a cabinet and on the driver's side is a good place to start.  You will likely have success from a number of locations so you will have choices.
